By taking this short survey, you will learn what residents identified earlier this year as the leading health problems, unhealthy behaviors, social issues, and service issues in our county.
You will also be informed about the leading causes of death for Person County according to the most recent state data. Your input will help us narrow down the top 2-3 health priorities for the county for the next 4 years.
You MUST BE 18 or older to complete the survey. You MUST ALSO LIVE AND/OR WORK in Person County to complete the survey.
